My bad, didn't pay attention to your subject. Can you check if the module is not missing at root level both static and defaultdoc and your web.config didn't do anything funny, also i have seen once...

The DefaultDocumentModule works if it's in a sub-folder. http://localhost/Test/ --> no problem http://localhost/ --> error The Default.aspx document is the same in both folders.
